Residential Case Manager

We are hiring a full-time case manager position (M-F 8AM-4PM) for our men's residential substance abuse program. Unfortunately, we do not provide any health benefits (insurance) or retirement benefits (401k), but we do offer vacation and paid time off benefits.
Responsibilities include but are not limited to:

    1. Intakes: Coordinating and complete intake and orientation for consumer upon admittance. Complete intake documentation and intake progress note in AccuCare. Work with the administrative assistant after the intake for scheduling of appointments.
    2. File Maintenance:  Closing residential files in compliance with accreditation, licensing, and contractual requirements.
    3. Clinical Authorizations: Completing admissions, discharges, initial authorizations, and re-authorizations in ELMER in a timely fashion by utilizing the treatment plan and individual session notes for all residential clients that are funded through NorthCare.
    4. Answering phone calls and providing any other assistance to the residential clients and the residential program
    5. Any other duty that promotes wellness of the person served, strengthens the organization’s mission, or promotes goodwill in the community.

 
QUALIFICATIONS:
Possess strong communication, organizational, computer, and team skills. Demonstrate a desire to help those with addictions. Baccalaureate Degree in social work, psychology, or other human service related field is preferred but not required. High school diploma or its equivalency is required.

Residential Support Staff

We are hiring for a part-time support staff position for Saturday and Sunday evening (4PM-12AM) in our men's residential program.
Responsibilities include but are not limited to:
 

  1. Dealing with emergency situations that may arise on the shift, taking action necessary to deal with the situation properly. Notify Executive Director, Clinical Supervisor, or Residential Counselors immediately.
  2. Medically monitoring according to Phoenix House Policy of consumer medications.
  3. Providing supervision to all clients, ensuring they follow the schedule, adhere to program rules and guidelines, and to include periodic room and building checks and following through with necessary interventions.
  4. Must be able to document legibly and inform other staff of issues that have taken place on shift.
  5. Shift worked will be 4:00 PM to 12:00 AM Friday and Saturday evening.
  6. Looking out for the consumers best interests, providing peer support, and keeping in mind at all times their health, safety, and welfare. 
  7. Providing transportation to scheduled outings (AA/NA meetings, Community Dinner, etc.).
  8. Must have yearly TB Testing and receive CPR and First Aid training (provided at no cost.)
  9. Any other duty that promotes wellness of the person served, strengthens the organization’s mission, or promotes goodwill in the community.               

                
 
QUALIFICATIONS: A high school diploma or its equivalency is required. A desire or passion to help those who are struggling with substance abuse. An individual who has good communication skills, problem-solving skills, boundary setting, and is able to be assertive when needed.

Outpatient Counselor

We are hiring for a part-time position located in our Ontonagon, MI office. Details are below:
Responsibilities of the outpatient counselor include but are not limited to:

  1. Completion and submission of all required documentation by the Department of Human Services, NorthCare, and any other entity that provides funding or referrals for services.
  2. Data entry into the computer systems which include all demographic and financial information necessary for outcomes management and billing purposes.
  3. Responsible for the intake paperwork, assessment, treatment planning, and discharge planning of the person served.  To include documenting progress, or lack of progress, on a daily basis.
  4. Responsible for the coordination of care necessary to bring stabilization to the person served which may include referrals to other agencies in the county where the client will be served.
  5. Providing individual therapy sessions for the person served, utilizing intervention strategies best suited for the client including, cognitive behavioral therapy, dialectical behavioral therapy, and motivational interviewing.
  6. Maintaining the client case file to include all documentation in accordance with organization requirements in an organized and professional manner.
  7. Maintain an outpatient counseling caseload.
  8. Actively engage in the staff development plan, striving to achieve objectives and goals, to become better at providing addiction services.
  9. Any other duty that promotes wellness of the person served, strengthens the organization’s mission, or promotes goodwill in the community.

 
QUALIFICATIONS:
Baccalaureate degree in social work, psychology, or human service related field.   Engagement in obtaining the certified addiction counselor status through a development plan or be a certified addiction counselor through the state of Michigan.  Experience or education within the substance abuse field is preferred. Ongoing education in all seminars and workshops related to substance abuse and outpatient treatment.
